Facilities Reservation Request

Please complete this form to request to reserve our facilities. Your reservation will  not be confirmed until you receive your confirmation email.

By submitting this form you agree to the following:

• All clergy other than our pastor must be approved by our pastor before performing any weddings and/or services.
• I have read the facilities use policy, and I agree to comply with the rules set forth. I agree that my deposit will not be returned if the facilities are not returned to the state in which they were found.
• I take full responsibility for the condition in which the facilities are left after my reserved time. Any damages that are incurred will be my sole responsibility. I agree to release Pelham First Baptist Church of any responsibility for any items brought to and/or left at the church by anyone associated with this events.
